Decorah Police Bias Incident Report Form

If you are visiting this page, you or someone you know may have experienced a bias incident, hate act, hate crime, harassment, or discrimination in the Decorah community.

In Decorah, acts of bias go against our mission and our community values. We seek to provide you a timely response and support. Bias refers to language and/or behaviors against persons because of, but not limited to, others’ actual or perceived: Age, color, creed, disability, gender, gender identity/expression, genetic information, national origin, race/ethnicity, religion, sex, sexual orientation, political affiliation, or veteran status.

Please provide as much detail about the incident as possible. Someone from the bias response team will follow up with you. Anonymous reports are accepted, but may limit the city's ability to fully address the incident.

Once submitted, the report goes directly to Assistant Police Chief Scott Herrmann (sherrmann@decorah.iowa.gov) and Captain Mandy Groth (mgroth@decorah.iowa.gov). The primary goal is to provide support to the individual or community affected. However, reports will be evaluated to determine if further investigation is required for potential violations of criminal law.

The response team will review the report and make a determination regarding its response. The response team is not responsible for adjudicating or sanctioning any individual or individuals named in the report. Rather, they will seek to advise the reporter of the options they have available through the legal processes. The team will follow-up to ensure a timely response has been received. The team may be directly involved when education and follow-up conversations may be requested or are a reasonable response. If you have questions, please contact one of the response team members.
